Referred by Your Dentist or Physician?
If your dentist or doctor has referred you to Concord Oral Surgery, please bring a copy of your referral slip and any recent X-rays or imaging to your consultation. These documents help us plan the most appropriate treatment for you.
If you would like your physician or dentist to refer you to our office, have them fill out our Online Patient Referral Form.
Accessibility and Parking
Our Vaughan office is located in a plaza with free on-site parking. The clinic is fully wheelchair-accessible. If you require extra assistance when you arrive, please let our front desk team know in advance, we are happy to help.
After-Hours Concerns
For patients who have recently had surgery and are experiencing unexpected symptoms or complications outside of office hours, please call our main number and follow the voicemail instructions for urgent post-operative concerns.
